Question about insulin treatment in a hospitalized patient without diabetes.?
People with hospital experience, what protocols have you seen in place for treating hyperglycemia in non-critical patients without a history of diabetes? Has anyone seen an intake blood glucose level of 120 trigger a decision to treat with insulin?
On a related note, does anyone know what would happen to a patient if you gave Apidra insulin outside of the reccomended time frame? Specifically the instructions are to administer within 20 minutes of beginning a meal. What would happen if it were given 40 minutes after the end of a meal? Again, this is in a patient who is not diabetic, and has, at most, only a slightly elevated blood sugar level.
Does anyone know of any hospitals where this is an accepted practice standard?
Sounds a bit weird to me....120 is NOT a bad BG level especially if they are ill. The time frame question would make me worry about HYPOGLYCEMIA. I would ask for a finger stick BG after the insulin (1 hour) just to see how low it is and go from there.
